JOB DESCRIPTION:

Daytop is looking for a Social Worker to join our team. This position under the direction of the Supervisor for Mental Health Services, the Social Worker provides family, individual and group therapy; evaluation of residents; discharge, and transition planning and monitors treatment planning efforts to assist the client in achieving substance abuse abstinence and mental health stability.

Principal Responsibilities:·

Performs intake, interviews and makes assessment of client`s suitability for treatment using relevant DSM diagnosis. Complete psychosocial and scheduled updates as required. Serve as a resource to clinical staff for crisis intervention.

·Develops appropriate treatment plans. Participates in multi-disciplinary treatment planning meetings.

·Make referrals as needed.

·Interacts regularly with external contacts (probation/parole services, court, social service agencies, employers etc.)

.Facilitates, organizes or supervises individual, group and family, HIV support therapy.

.Completes third party pay or forms required for re-imbursement. Pre-certify clients with appropriate insurer maximizing billing efforts.

.Participates in staff and supervisory meetings and case conferences.

.Prepares lessons or training in specialty area to be used for staff in-service training or discussions.

·May be required to work some weekends or provide coverage for other shifts within the facility as well as holidays and other special events hosted by the agency.

·All other related duties as assigned.

Qualifications:

LMSW required, LCSW preferred. 2-3 years experience and/or training or equivalent combination of education, training, and experience a must. Previous experience or knowledge of therapeutic community model helpful.
